Limitation of Liability Policy: I give permission for the listed student to participate in the Sports Concept Recreational programs and/or additional activities. This includes all on-ice and off-ice sessions and any other programs offered by Sports Concept Recreational. As a condition of participation in skating programs offered by Sports Concept Recreational, Sports Concept Recreational and associated staff are not liable for any injury, loss, or damage suffered by the above-noted student while participating in skating practices or other activities however caused. The member, or his/her parent/legal guardian who has signed this form, shall indemnify Sports Concept Recreational from any harm, claims, demands or actions arising from or in respect of such injury, loss or damage.
